Use FormBuilder from Claude, ChatGPT and other AI assistants

FormBuilder has a built-in connector (an MCP server) for AI assistants. Once connected, you can say "make me a customer feedback form and publish it" or "summarize this week's responses" and the assistant does it in your FormBuilder account.

Workflow

Steps

  1. 1In Claude: Settings → Connectors → Add custom connector → name it FormBuilder and paste https://api.formbuilder.com/mcp → Connect. (Coming to the Claude connector directory as a one-click install.)
  2. 2You are sent to formbuilder.com to sign in (or sign up — it is free) and asked to Allow. Nothing else to configure.
  3. 3Back in the chat, turn the FormBuilder connector on and ask for what you want: "Create a job application form with name, email, position and resume upload, and publish it."
  4. 4The assistant replies with the live form link and an edit link. Ask follow-ups like "list my forms", "what fields does it have?", or "show me the latest 10 responses and summarize them".
  5. 5To disconnect, remove the connector in the assistant, or go to Settings → API keys in FormBuilder and revoke the key named "Claude (OAuth)" (or ChatGPT, Cursor, …).

What the assistant can do

  • Create a form from a description and optionally publish it (same safety review as the app — forms that collect passwords or card numbers in plain fields are held).
  • List your forms with status and response counts.
  • Look up a form's fields.
  • Read responses to a form and summarize or tabulate them (held over-limit responses show as locked until you upgrade).
  • Register a webhook so new responses are POSTed to a URL — only if you explicitly allow the "webhooks" permission on the consent screen.
  • It cannot delete forms or responses, change billing, or touch other accounts.

Other assistants and tools

  • ChatGPT: Settings → Apps & Connectors → add https://api.formbuilder.com/mcp (Developer Mode may be required until the app is listed).
  • Claude Code: run claude mcp add --transport http formbuilder https://api.formbuilder.com/mcp
  • Cursor / Windsurf / Claude Desktop: add an MCP server with command npx and args -y formbuilder-mcp, and set FORMBUILDER_API_KEY to a key from Settings → API keys.
  • Gemini CLI: gemini mcp add formbuilder https://api.formbuilder.com/mcp

Security and privacy

  • The assistant never sees your FormBuilder password. It receives a scoped token that expires after 30 days and appears under Settings → API keys, where you can revoke it any time.
  • On the consent screen, known clients (claude.ai, ChatGPT, Cursor, Windsurf, Gemini) are marked "Known client"; anything else shows an "Unverified app" warning — only allow apps you recognise.
  • Responses the assistant reads are sent to that assistant's provider under their privacy terms. FormBuilder never uses your form data to train AI models. See our privacy policy, section "AI Assistants, MCP Server and Connected Apps".
